When exposed to antigens, histamine is released, triggering allergic reactions. This mechanism plays a role in directing Neutrophil and Eosinophil to affected areas, though many aspects remain unknown.

The character appearing in episode 5 is a man covered except for his eyes, carrying an umbrella, wearing a black mask, a blue knit cap, and a blue-green raincoat. He is a mysterious figure who uses convoluted expressions that are hard for Neutrophil and Mast Cell to understand, though Eosinophil comprehends him. He remains calm even in the face of incidents.

In episode 27, Basophil's childhood is depicted, revealing his handsome face. As a senior to White Blood Cell U-1146 and Eosinophil, he is someone they admire and who has greatly influenced them. He had been reading the poetry collection "Tale of Fine and Cell" from a young age and tends to use complex expressions, though he also shows a naïve side.

After the left shift, his whereabouts become unknown. Since Basophil accounts for less than 1% of all White Blood Cells, White Blood Cell U-1146 and the others believed for a long time that he had died.
