Esou is the second of the three brothers born from the Cursed Womb: Death Painting, alongside his elder brother Choso and younger brother Kechizu.
He becomes involved with Suguru Geto (the impostor using his body) and Mahito, cooperating with their plan to eradicate humanity and create a world ruled by cursed spirits, though he mostly follows them because of Choso.
After being taken from the Tokyo Jujutsu High vault and incarnated, Esou is dispatched with Kechizu to retrieve one of Sukuna Ryoumen’s fingers.
During this mission at Yasohachi Bridge, he encounters Yuji Itadori and Nobara Kugisaki and dies in battle against them.
Esou is best described as a half-human, half–cursed spirit with a surprisingly gentlemanly demeanor.
Despite his grotesque abilities and appearance, he speaks politely, avoids pointless fights, and will refrain from killing sorcerers if he sees no need.
He is calm, collected, and even offers enemies the chance to walk away if a clash seems unnecessary.
However, this composure collapses when someone sees his back or harms his brothers, at which point his speech becomes rough and he flies into a rage.
Esou has no personal grudge against human beings or sorcerers in general.
He lacks memories of his mother and does not naturally harbor hatred toward humanity, but his view is warped by his grotesque birth and his deep bond with his brothers.
His love for his siblings is absolute and defines him.
Esou believes that if it is for his brothers, and if his brothers desire it, he will dedicate and sacrifice himself without hesitation.
He constantly worries about Kechizu in battle, coordinating with him and prioritizing his safety over tactical advantage.

When Kechizu is killed, Esou is devastated, crying openly and apologizing in his heart to Choso for failing to protect their younger brother.

He has a muscular build, dark tan skin, and a black mohawk hairstyle, giving him a punk-like presence.
He usually appears barefoot and wears a bow tie and a feminine body harness over his bare torso, along with black leg warmers and a T-back style undergarment.
His face is actually quite handsome if you ignore the outfit, though he does not resemble Choso very closely.
His irises are purple, while the whites of his eyes are entirely black, enhancing his inhuman aura.
When he appears in the original manga, the background often features rose motifs and a flamboyant “fao”–like sound effect to emphasize his eccentric vibe.
In the anime adaptation, his extremely revealing fashion was toned down.
The T-back was replaced with tight, leggings-like pants, likely to pass broadcast standards while keeping his weird, fetish-like style.
The most disturbing part of his body is his back.
There, he has a grotesque second face that constantly sheds bloody tears, emitting a rotten, pus-like stench.
