As a child, Juuzou was kidnapped and raised by Big Madam, a ghoul who used him as a “pet” and “replacement daughter.” His birth name is “Rei,” but Big Madam called him “Juuzou,” a name he kept.
Enduring years of torture and psychological manipulation, he suffered mutilation, including the destruction of his male genitalia to maintain his “cuteness.” The trauma dulled his pain receptors and fractured his psyche, at one point developing symptoms of dissociation.
Eventually abandoned by Big Madam, Juuzou was taken in by the CCG, but struggled socially and morally. Tokyo Ghoul:re – Turning Point

During the Nutcracker case, Juuzou cooperated closely with the Quinx Squad, infiltrating a human auction and ultimately defeating Big Madam—his abuser—personally, which propelled him to Special Investigator. In the Rushima Landing Operation, he led the Second Squad, subduing half-ghouls like Kurona Yasuhisa, and withstood severe injuries during the final conflict with Ken Kaneki in “dragon” form, but ensured his squad’s survival.
Post-War and Growth
After the conflict, Juuzou accepted Itsuki Marude’s proposal to cooperate with ghouls, marking a major evolution in his worldview.
He was appointed as the highest-ranking “Dragon General” of the new organization TSC and wept at the revival of his mentor, Yukinori Shinohara.
