Vaccine Man is outraged that someone with such a frivolous reason would dare stand in the way of an “apostle of the Earth.”

Infuriated, he transforms into his enormous, monstrous true form to annihilate Saitama.

However, before Vaccine Man can even begin his assault in earnest, Saitama throws a single punch.

Vaccine Man is completely obliterated in an instant, turned into flying chunks and dust.

The King Misunderstanding

In a later side story illustrated by Yusuke Murata, titled “Witness,” more detail is added around the aftermath.

It is revealed that 31 A-Class and lower-ranked heroes arrived at the scene before or during Vaccine Man’s rampage and were casually taken out by the collateral damage of his attacks.

After Saitama kills Vaccine Man, King comes to the ruined area simply to retrieve a game he dropped earlier.

While walking through the rubble, King trips on debris and falls face-first into Vaccine Man’s blood.

Rescue heroes and other witnesses then arrive and see King standing there, covered in blood and surrounded by the remains of Vaccine Man.

Based on appearances alone, they conclude that King is the one who defeated Vaccine Man.

As a result, official records and public perception credit King as the hero who destroyed a Dragon-level monster in City A.

This misunderstanding quietly contributes to King’s undeserved reputation as the “strongest man on Earth,” while Saitama remains unrecognized.

Fans have noted that Vaccine Man’s energy manipulation is very similar to that of Homeless Emperor, a later monster who uses “divine” energy orbs granted by a mysterious being often referred to as “God.”

Both characters conjure floating spheres of destructive energy, hurling them as long-range devastation.

According to Yusuke Murata, the artist of the remake manga, the energy blasts used by Vaccine Man, Homeless Emperor, and Choze are of the same type.

This has led to speculation that all three may be tied to the same underlying power source, possibly that “God-like” entity.

However, it is unclear whether this connection and worldbuilding detail originated from the original creator ONE or is a later interpretive layer.

As of the information available in the source material, the exact canonical relationship between Vaccine Man and “God” remains unconfirmed.

In the anime adaptation, Vaccine Man appears in Episode 1.
