The Cancer Cell Boss appears in episode 8. Harboring resentment over the unreasonable fate of being killed by immune cells, he plots to take over the body. He disguises himself as a General Cell but is exposed by the NK Cell. Empowered, he gains the upper hand in battle against the NK Cell, White Blood Cell U-1146, and Killer T Cell. However, he is ultimately defeated by reinforcements summoned by Erythrocyte. In his final moments, he laments his origins, saying, "I was just born..." and expresses a bit of joy at being treated gently by White Blood Cell. As he dies, he declares he will return.

In episode 22, due to a cellular information leak, he is resurrected and manipulates the Regulatory T Cell to once again attempt to conquer the body. He tries to annihilate the immune cells by using toxins from a Pathogenic Bacteria colony, but thanks to the efforts of the Lactic Acid Bacteria, the toxin supply is cut off. He is blown away by Memory T Saibou and has his throat cut by White Blood Cell U-1146, dying once again. He leaves the words, "I hated Killer T and NK, but I actually liked you," behind.

In episode 2 of the anime, it is revealed that the General Cell who helped Erythrocyte escape from bacteria that invaded through a scratch was actually him.
