Former Baroness Barnett. A female doctor working at the Royal London Hospital. She is Rachel's sister and Ciel's aunt. A flamboyant beauty with red hair inherited from her father, she prefers to wear red and is known as Madam Red. She is also called "The Belle of Society" and "The Queen of the Night" due to her social activities. She adores Ciel, the son of her beloved sister, as if he were her own, and is an ally of the Phantomhive family. However, she was actually the true identity of Jack the Ripper, who caused a stir in London.

Originally a plain and simple girl with a complex about her red hair, which she hated. However, she started to like the color red and gained confidence in herself when Vincent, who began visiting her home, complimented her red hair and she developed feelings for him. She was shocked when Vincent married her sister, leading her to despair and to attend social gatherings in flashy makeup and bright red dresses, becoming known as Lady Red and a notable figure in society. While secretly jealous of her sister who married the man she loved, she later met and married Baron Barnett, a kind man who accepted that she had loved someone else, and became pregnant with his child. However, she lost her husband to a carriage accident, and a surgical procedure to remove her uterus and fetus rendered her unable to have children again. The attack on the Phantomhive family that resulted in the loss of her sister and her brother-in-law, and her nephew's disappearance, left her feeling void of everything important to her. In her work as a doctor, her intense anger towards prostitutes who casually sought abortions, despite her own inability to have children, led her to murder them one by one, taking their uteruses in a series of bizarre killings. This is how she encountered the reaper Grell Sutcliff, and ostensibly employed him as a butler to carry out her murderous spree. This backstory reveals that the murders began while Ciel was missing.

In the "Jack the Ripper Arc," despite being the culprit, she cooperates with Ciel and Lau in the investigation. In the climax of the arc, when Ciel and the others close in on the truth, she attempts to kill Ciel but hesitates at the last moment, unable to kill her nephew who reminds her of her beloved sister.
